Senior Product Engineer (US)Up to $190,000
About the Role
Gaia bundles IVF, egg freezing and embryo transfer treatment into one fixed price and takes on the financial risk itself, so members who don't get a successful outcome don't pay in full. You'll work across the Orchestrator - their internal framework automating care navigation, financing, clinic operations and risk tracking across a web of patients, clinics, lenders and insurers - alongside a real-time underwriting engine that prices treatment against predicted outcomes, and an AI-native operations layer where LLM-powered agents run production workflows rather than sit behind a chat window.
This is a genuinely cross-domain build: healthcare data that was never designed to talk to lenders or insurers, financial risk models with real edge cases, and HIPAA and SOC 2-level constraints treated as first-class engineering concerns rather than afterthoughts. You'll own real slices of this system - state machines, workflows, and integrations between systems that weren't built to cooperate - rather than just shipping features around the edges of someone else's architecture.
What They're Looking For
- You bring 5+ years as a full-stack engineer in startup environments, ideally with most of that tenure at a single company you helped take from zero to one and beyond
- You're backend-leaning in Python, comfortable owning state machines, workflows and integrations across systems that weren't built to talk to each other
- You've already made AI tools like Claude Code, Cursor or Codex a core part of how you build - not something you're curious about, something that's measurably changed your workflow in the last year
- You've worked somewhere that treated regulatory or safety constraints - HIPAA, SOC 2, PCI, or similar - as a first-class engineering concern rather than an afterthought
- You'd rather ship a simple solution than build a clever one, and you're comfortable taking on loosely-defined problems and breaking them down yourself
